SECTION 3 – Reasons for application
Please provide a personal statement describing why you would like to join the 6th form at Whitchurch High School, what you would hope to achieve, what your contribution to the 6th form and the school will be, and what your interest/hobbies are. Please limit this to one side of A4 and attach to this application form. Students from schools which offer the Skills Challenge Certificate (Welsh Baccalaureate) will be expected to incorporate information about their achievements in this subject area at key stage 4.
If you wish to join the rugby academy at Whitchurch High School, you will also need to provide a brief letter outlining your rugby playing experience to date.

SECTION 4 – Option Choices (see following page for the option grid).


  • Select one subject per column and record your choices in the space provided at the bottom of each option column. You will be expected to choose 4 subjects in Year 12 (from columns 1, 2, 3 and 4), with the option to reduce to three subjects in Year 13.

  • You will follow the Advanced Skills Challenge Certificate (Welsh Baccalaureate) throughout Year 12 and Year 13. This is compulsory.

  • If you wish to be considered for the Rugby Academy at Whitchurch High School, please enter Rugby Academy in the space at the bottom of option 5.

  • We cannot guarantee that all courses listed will run and some courses my run in collaboration with other schools.

  • We cannot guarantee that you will get a place on your chosen course. Please refer to the admission requirements document attached for specific details on entry requirements and the allocation of course places.

SIXTH FORM ENTRY REQUIREMENTS

You will need:

  • A minimum of 6 full course GCSEs at grade C or above* to study four AS-levels in year 12 and the compulsory Advanced Skills Challenge Certificate (Welsh Baccalaureate).

  • A minimum of 5 full course GCSEs at grade C or above* to study three AS-levels in year 12 and the compulsory Advanced Skills Challenge Certificate (Welsh Baccalaureate).


*Grade C is the minimum requirement. Some subjects require a minimum of Grade B at GCSE to continue your studies at AS/A2-level. You can find the minimum requirement for each subject area in Table A on pages 3 & 4. Specific details about individual courses and entry requirements are available on our website at:

Please note that where the minimum requirement for a course is grade B some students with grade C may be accepted onto the course. We will make decisions on suitability based on prior performance, aptitude and work ethic.

Allocation of Course Places

Where the minimum entry requirement for a course is listed as a B grade:

Places will be allocated as follows:

  • 1st allocation: Those who have met the entry requirement and applied to study the course by the initial deadline for application forms to be submitted.

  • 2nd allocation: Those who have met the entry requirement.

  • 3rd allocation: Those who achieved a C grade and have been deemed suitable by departmental staff based on prior attainment, aptitude and work ethic and applied to study the course by the application deadline.

  • 4th allocation: Those who achieved a C grade and have been deemed suitable by departmental staff based on prior attainment, aptitude and work ethic.
