FINANCIAL AID GUIDELINES

The purpose of the Maine Gladiators Scholarship Program is to enable a child to experience the game of hockey who might not otherwise be able to afford the cost of the program. The Gladiators are able to provide financial aid on a year to year basis based on the financial condition of the Association with the following guidelines in effect. It is not the intent of the program to provide for 100% of the cost, but to partially offset the costs.
The Maine Gladiators Scholarship Committee is comprised of the Treasurer, two additional board members appointed by the President. Applications are considered confidential and only shared within the confines of this committee and for financial record keeping purposes. All records shall be destroyed after decision is made. The decisions made are done within the complete discretion of the Scholarship Committee.
Guidelines:

- Financial aid can take the form of scholarships or payment plans.

- All financial aid requests must be submitted by August 1st.

- 100% scholarships will not be provided. Scholarship amount will be determined

on an individual basis, depending on applicant’s financial circumstances.

- Amount granted will not exceed two thirds of the assessment fee. For example, if

the fee is $1,000 the player receiving the maximum scholarship would pay

$333.00.

- Each skater, whether applying for a scholarship or not must pay the commitment

fee of $250.00.

- A new application must be submitted with each new season.

- Scholarships will only be granted for a maximum of two years of play within

Gladiators.

- Parents who receive financial aid are expected to commit a minimum of 10 hours

volunteering for Gladiators. Volunteer opportunities will depend on Gladiators

needs and unique skills to be offered. Examples include assisting at Gladiators

Tournaments, serving as team administrator, sewing jersey patches, helping with the golf fund-raiser, etc.

- Scholarship requests will be limited to one full scholarship (2/3 reduction) for the

first child and a half scholarship (1/3 reduction) for the second.

Each family requesting financial aid must complete the attached application in full and return it with supporting financial documentation (a copy of your most recent tax return – 1040 and all related schedules) to the Maine Gladiators Scholarship Committee for review. If parents are separated or divorced, each parent must provide documentation.

Please mail this form to Maine Gladiators, PO Box 7265, Lewiston, ME 04243-7265. The application must be completed in full. Please attach a copy of your most recent tax return (1040 and all related schedules and business/entity schedules or returns). If your financial situation has changed since that time, please note so when explaining reasons for request.
